Job Summary
This position is responsible for the oversight and provision of behavioral health services and supervision of student interns in assigned locations. This position will be expected to adhere to the NASW Code of Ethics and comply with HIPPA regulations. Reports to the Principal Investigator.
Primary Duties and Responsibilities
+ Manage operations of social work services within Forrest Health's Family Medicine Residency Clinic (FMRC) to include required documentation and reporting to contract agency and appropriate leadership.
+ Collaborate with patient care team to identify and connect with patients in need of behavioral health services and provide behavioral health consultation to medical providers.
+ Assess patient's progress and future needs, assist patients with adherence to medical treatment plan, and coordinate referrals other community resources as needed.
+ Provide ethical behavioral health interventions, grief counseling, and support for substance abuse issues to clinic patients.
+ Utilize evidence-based practices and clinical care management treatment strategies with patients and their families.
+ Provide clinical supervision and management of assigned student intern schedule.
+ Document all patient contacts and communication in compliance with program protocols within the electronic health record.
+ Performs other duties as assigned.
