Job Title:

RN Patient Flow Coordinator-Discharge Lounge

Job Summary:

Maintain direct control of hospital beds by actively managing patient discharges, following up promptly, and ensuring discharged patients are consistently and quickly removed from the EHR for timely bed reassignment.

Education & Experience

Education and Experience Required:

 

One (1) year of nursing experience in an inpatient setting.

 

Certifications, Licenses, or Registration required:

 

Valid RN license

 

Preferred Qualifications: 

 

RN case management certification preferred

 

Knowledge, Skills & Abilities

Knowledge, Skills, and Abilities:

 

Knowledge of care management and resource/utilization management. Skill in the use of personal computers and related software applications. Ability to manage multiple priorities under time constraints; ability to analyze and solve problems. Understanding cost and quality issues. Verbal and written communication skills. Interpersonal skills to interact with a wide range of constituencies. Decision-making skills.

 

Responsibilities:

 

Review previous day’s discharges and round on patients scheduled for discharge to identify and address barriers.

Encourage and facilitate discharges prior to noon to optimize patient flow and ensure timely bed availability.

Collaborate with clinical and non-clinical teams to mitigate discharge hurdles and prevent delays.

Ensure all discharges occur safely, efficiently, and in compliance with hospital policies and CMS regulations.

Authorize patient transportation as needed to support timely discharge.

Maintain expertise in discharge processes, serve as a resource, and educate staff on best practices.

Promote teamwork across departments to ensure smooth patient transitions.

Monitor discharge efficiency, productivity, quality, cost, and patient length of stay, identifying at-risk populations.

Drive high levels of satisfaction for patients, families, staff, and internal stakeholders.

Physical and Environmental Demands

Requires occasional exposure to unpleasant or disagreeable physical environment such as high noise level and exposure to heat and cold, occasional exposure to biohazardous conditions such as risk of radiation exposure, blood borne pathogens, fumes or airborne particles, and/or toxic or caustic chemicals which mandate attention to safety considerations, occasional working hours significantly beyond regularly scheduled hours, occasional travelling to offsite locations, occasional work produced subject to precise measures of quantity and quality, occasional bending, occasional lifting and carrying up to 25 pounds, occasional crouching/stooping, occasional kneeling, occasional pushing/pulling, frequent reaching, frequent sitting, frequent standing, occasional twisting, and frequent walking. (occasional-up to 20%, frequent-from 21% to 50%, constant-51% or more)
