  Job Responsibilities:

  
  
  
  
  • Responsible for delivery of social services to residents and families
  
  
  • Handle pre-admission and admission interviews, informs resident and family of available services and resident rights and responsibilities
  
  
  • Prepare social histories for use by professional facility staff and participates in resident care conferences
  
  
  • Handle problems of adjustment to the long term care facility environment, maintains frequent contact with residents and families and serves as an advocate for patients
  
  
  • Establish working relationships with community health, welfare, and volunteer groups
  
  
  • Assist with arrangements, if needed, for out of facility health care, transportation, and counseling services
  
  
  • Assist residents and families with transitions in and out of the facility, connecting them with needed services 
  
  
  • Maintain accurate records, ensuring compliance and proper tracking of resident progress and services
  
  
  
  

  Qualifications:

  
  
  
  
  • Bachelor's Degree in Social Work and at least one year's experience in Long Term Care
  
  
  • Strong communication and interpersonal skills
  
  
  • Understanding of healthcare regulations and resources
