CASE MANAGE

CASE MANAGER (Full Time)

DUTIES AND RESPONSIBILITIES INCLUDE BUT ARE NOT LIMITED TO:

A.      Case Manager will manage their work schedule in order to provide optimum interaction with inmates on your caseload

B.      Case Manager will assume sole responsibility for working file security

C.      Case Manager will maintain in a three-ring binder a log-in form for all inmates to sign and it will be available for supervisor review

D.     Case Manager will assume responsibility for scheduling interface sessions at a minimum of (1) contact per month

E.      Case Manager will record all contacts on Offendertrak the same day that said contact takes place

F.       Case Manager will maintain working and electronic files on inmates

G.     Case Manager will be responsible for accuracy and appropriateness of classification status and annual reclassification reviews by reviewing inmates NCIC Rap Sheets

H.     Case Manager is responsible for submitting addresses in an appropriate time frame for parole and ERS release and submitting Interstate Compact applications within 90 days of release

I.        Case Manager will answer inmate requests in a timely manner but no more than (10) days from date of request

J.        Case Manager will act as the liaison between free world people and the inmate

K.      Case Manager is responsible for TET, MET, parole plans, etc.

MINIMUM QUALIFICATIONS

A.      Possess a high school diploma or equivalent

B.      Must be proficient in computer knowledge

C.      Have two to five years’ experience in an office environment

D.     Must be able to communicate directly with inmates and family

E.      Ability to exercise initiative, judgement, and know how in the performance of complex and confidential duties

F.       Education may substitute for some experience
