A Home and Community Support worker at Divine Behavioral Solutions plays a crucial role in working with individuals and families to provide a service to make their lives easier. HCS workers will collaborate with parents according to the needed times of the service to provide assistance with daily living ADLs and provide community integration.

Duties and Responsibilities: • Providing support and care to adults with disabilities . • Document notes per shift and implement services tailored to the Activity Support Plan • Expected to be prompt and present during shift at all times. • Assist with activities such as eating, bathing, grooming, toileting transferring and mobility. • Responsible for vssitaice with incidental cleaning related to the persons health. • Engaging with individual through community activities tailored to the person’s interest by providing transportation . • Monitor the individual at all moments be detail oriented to their safety. • May assist a person with shopping with authorized consent from legal guardian. • Develop and follow recommended teaching techniques through collaborations of those associated with the individuals plan of care. • Report any immediate safety hazards to appropriate supervisory personnel.
Requirements • Required one year of working experience • One year of working experience with Adults with Developmental Disabilities preferred. Education Required • Must be at-least 18 years old High-school diploma or GED equivalent and have valid drivers license with valid vehicle insurance
