Social Worker
Minimum Qualifications: Master's Degree in social work from an accredited school of social work. Board Certified, Credentialed or eligible for credentialing with privileges in social work. One year experience in a psychiatric setting or in a social agency. Prefer experience with Medicaid and Medicare payment system. Excellent written, verbal and interpersonal skills.
Summary of Major Functions: The Social Worker promotes and maintains the delivery of high quality mental health care services for patients and their family members. Plans, organizes, directs, defines, and controls social work clinical services necessary to meet the legal, organizational, medical staff and regulatory guidelines. The Social Work services are structured to ensure that the discipline of social work is represented within the clinic’s treatment protocol and must be able to function effectively as a member of the treatment team.. The Clinical Social Worker is responsible for providing the following services: Psycho-Social History and family contact; primary therapeutic services/ therapy such as group therapy, individual therapy, family and marital therapy, crisis intervention services, and education; discharge planning and community referral. The Social Worker participates as a member of the multidisciplinary treatment team, and is
responsible for assessing, planning, implementing, and evaluating all phases of the treatment planning under the supervision of the
Behavioral Health Clinic Manager. This includes performing an essential function in the interdisciplinary treatment process, by organizing and facilitating individualized treatment plans and reviews. Maintains patient confidentiality at all times, and documents according to policies and procedures. The Social Worker participates in department meetings, Quality Assurance process and case reviews. The Social Worker participates in community education activities, works with referral sources for the continuing care of each patient, and has (or develops) an excellent knowledge base of community resources for continuing care. Must be able to manage and prioritize time to meet the needs of the patients and family. Attends all required in-services and continuing education.
